Synopsis

Antimicrobial resistance constitutes one of the major threats to global public health, driven by the indiscriminate use of antibiotics and the evolutionary adaptability of microorganisms. This chapter presents a systematic review of next-generation antibiotics as key tools against multidrug-resistant bacteria. Thirty-six recent articles (2020–2025) were analyzed, evaluating new β-lactam/β-lactamase inhibitor combinations, siderophore cephalosporins, and innovative drugs such as lefamulin, delafloxacin, tedizolid, and long-acting lipoglycopeptides. The results demonstrate significant improvements in clinical efficacy, pharmacokinetics, and safety, as well as reduced mortality in nosocomial and complicated infections. Furthermore, advances in the treatment of drug-resistant tuberculosis with bedaquiline, delamanid, and pretomanid are discussed, along with the importance of antimicrobial stewardship programs (ASPs) in containing emerging resistance. Finally, the need to integrate therapeutic innovation, rapid diagnostics, and microbiological surveillance is highlighted as an essential foundation for ensuring antibiotic effectiveness in the coming decade.
