Synopsis

Introduction: Surgical site infections continue to be a relevant cause of postoperative morbidity. Surgical antibiotic prophylaxis is an effective measure to reduce them, provided that it is based on scientific evidence. Methodology: Systematised narrative review (2020–2025) in PubMed, Scopus, Web of Science and Cochrane, focussed on efficacy, timing of administration, type and duration of the antibiotic. Results: Cephazoline was the most effective agent; intraoperative redosification and suspension in less than 24 hours decreased the incidence of infection and bacterial resistance. Discussion: The improvement of PAQ requires continuous education, institutional audits and antimicrobial stewardship programs that optimise surgical safety.
